Here’s what needs to go in the incident report form:

Who was involved?

Please include names, chapter affiliations, WSU ID's, and any other meaningfully identifying information

If you are identifying a chapter, please indicate what lead you to believe it was the chapter you identified

Describe what happened:

Being detailed and specific will help us more effectively adjudicate the issue

Were the police involved? If so was there a Police Report?

If you have photos or videos please attach them.
